The decision to go home is one of many steps you’ll
take on the road to recovery.
The transition-to-home process started upon admission,
so our Discharge Planner has already been hard at work to match your needs with
any available community resources to help ensure your transition is smooth and worry-free.
Our Discharge Planner lines up the services you’ll need at home, among them being
Home Health Care services, hospital equipment, Meals on Wheels, adult day care,
and waiver application. Plus, our Discharge Planner will provide you with information
about a host of other community resources and programs you may wish to consider
once you’re back home.
Patients
who are discharged home following rehabilitation will also be invited to a special
reunion luncheon for rehab “graduates” at a local restaurant to renew acquaintances
and share their experiences.
We Make House Calls!
Prior to your discharge, you may
take advantage of a free “house call” in which one of our therapists accompanies
you to your home for a brief assessment of the layout of your living quarters. The
therapist observes you move about in your home environment and identifies any issues.
For instance, a therapist may suggest you replace a throw rug with a mat with a
non-skid backing to prevent a slip and fall, or the therapist may recommend having
grab bars installed to assist you with your bathing. The therapist’s evaluation
and recommendations will make you feel comfortable in your home and confident that
your transition will be successful.
